Go to the Jacksonville Housing Authority at 1300 Broad Street, 2nd floor. (Have your photo ID and Social Security card with you.)

 

 

Request to apply for Liberty Center for Men or The Liberty Center for Women. (Anyone convicted of a felony in the last 5 years must be released from incarceration for at least 2 years without a re-arrest for similar charges. HUD does not accept sex offenders or predators.)

 

Each applicant will receive a completed Certificate of Homelessness from HUD, which has to be completed by a local shelter such as:

 

   - Trinity Rescue Mission – located at 622 W Union St. – (904) 355-1205

   - City Rescue Mission – located at 234 W State St. – (904) 353-8561

   - I.M. Sulzbacher – 611 E. Adams St. – 359-0457, ext. 281

 

Upon completion of the certificate of homelessness, the applicant will go to the respective property to fill out a Liberty Center application and schedule an interview.

 

We interview Mon-Thurs from 8:30 – 11:00 p.m.

 

Please bring with you a Photo ID, SS Card and all paperwork given to you by JHA.

To apply for liberty center iv, you must come to the property to apply in person.
